(This plan was not written by an epidemiologist or public health pro, so everything here should be taken with a mountain's worth of salt. This is not medical advice and I am not a physician.)

Depending on outcomes of pilot efficacy tests, until community spread has stopped or a vaccine is available, everyone for whom it is safe to do so begins taking a prophylactic dose of hydroxychloroquine (dosage TBD). Purpose of prophylactic dose is to reduce early-stage viral load, slowing illness progression and reducing transmission.

Phase 1

... lasts until: everyone who lives or works with a vulnerable individual has been tested.

• Focused aggressive testing. Everyone who works or lives with members of the most vulnerable populations gets tested. Tests are followed up with contact tracing as much as possible.

• Everyone stays home, except for groceries, etc. If people do go out, N95 mask/respirator, glasses, and disposable glove wearing is mandatory.

• Clear communication of instructions for indoor + outdoor wear, and for decontaminating items brought into the home (e.g. spray all amazon purchases, groceries, with 70% rubbing alcohol).

  Costs:

  • paused economy
  • N95 masks/respirators for everyone (1 disposable per person per month). ~300M masks/respirators per month. N95 masks/respirators get decontaminated (by placing in oven at low heat) and reused until new mask/respirator is issued
  • vulnerable populations and those who live+work with them (guessing ~20% of US population, or 66M tests)

Phase 2

... lasts until: r0 < 1 in a locality

• Schools, restaurants, bars, and other social venues remain closed

• Aggressive testing of all who have symptoms, and anyone who can pay for it. Test is free for those who test positive.

• Aggressive contact tracing and quarantining.

• Everyone wears an N95 mask/respirator (people get 1 per person per month, which they reuse) when they are outside in a crowded environment, in any building that's not their home, or in any car that's not their's.

• Vulnerable populations remain isolated, as should those who work with vulnerable populations.

• Those who work with vulnerable populations should be tested regularly if possible.

  Costs:

  • closed schools, restaurants, social venues
  • N95 masks/respirators
  • isolation of vulnerable population
  • regular testing of those who work with vulnerable populations

Phase 3

... r0 must stay < 1 until eradication or steady state

• Movement between "green" (r0 < 1) regions is permitted. Movement out of "red" (r0 > 1) regions is restricted.
• Continuous random sentinel testing in top 20 US markets. Less frequent random testing in smaller towns. ****
• Schools, bars, restaurants reopen locality by locality, depending on active monitoring of local cases
